Free initial phone conversation

Currently I am contracted with Premera Blue Cross, LifeWise Health Plan of WA, First Choice, Kaiser PPO  and Cigna. I can also bill as an out of network provider for most other insurance companies. As a licensed clinical social worker, I am able to provide you with the necessary paperwork for you to submit to your insurance company for my services as an out of network provider. I am also able to bill electronically for you as an in or out of network provider. I recommend you contact your insurance company prior to our first visit to ascertain if they will offer this benefit and what percentage they will reimburse you.  Adjusted fee scale available on a case by case basis.
